  • Baptismal Certificate (In case Birth Certificate is unavailable)

  • 1 x 1 I. D. Picture (2 pcs)

  • For A&E Elementary - Elementary undergrad - 13 years old and above

  • For A&E Junior High School (JHS) - Secondary undergrad - 16 years old and above
